A South Korean city is embarking on an unprecedented experiment to accelerate medical AI development by transforming its entire urban landscape into a real-world testing environment. Wonju, located in Gangwon Province, is positioning itself as a national hub for medical AI commercialization through the "Gangwon Medical and Wellness AX Transformation Project," which proposes integrating AI validation directly into the city's hospitals, residential areas, and healthcare facilities.
The initiative draws inspiration from Toyota's Woven City concept, creating what officials call a "Korean-style" model where urban infrastructure doubles as a demonstration stage for developing, validating, and commercializing medical AI solutions before global expansion.
A Fast-Track System for Medical AI
At the heart of Wonju's strategy is the "Medical AX Commercialization Fast-Track System," designed to connect data utilization, validation, regulatory approval, and commercialization in a single streamlined pipeline. The project leverages Wonju's existing strengths: over 30 years of medical device industry expertise, access to National Health Insurance Service health data, and collaboration with the Health Insurance Review and Assessment Service.
According to the project timeline, master plan development is scheduled for 2026, followed by a pre-verification phase in 2027 and full launch in 2028. The initiative secured 1 billion won (approximately $707,000 USD) in December 2024 for preliminary feasibility planning in 2026. A specialized agency was selected in June 2024 to develop the master plan, with a final planning report targeted for October 2024.
The project agreement was formalized in April 2024 between the National IT Industry Promotion Agency and Wonju Mirae Industry Promotion Agency, signaling coordinated support across national and local government levels.
Government and Industry Alignment
The initiative has attracted high-level political backing. Participating officials include Rep. Song Ki-heon, Chairman of the National Assembly Science & Technology Committee; Deputy Prime Minister Bae Kyung-hoon, who serves as Minister of Science and ICT; and Rep. Lee Gwang-jae, Chairman of the Budget Committee. Wonju Mayor Koo Ja-yeol is also among the key supporters.
A project briefing is scheduled for Thursday in Seoul, bringing together government ministries, National Assembly members, local governments, and industry groups to coordinate implementation strategies.
Implications for the Medical AI Ecosystem
For healthcare providers in Wonju, participation means hospitals and care facilities will serve as integrated validation sites, potentially offering early access to emerging AI technologies while generating real-world evidence for regulatory approval. Health tech companies gain access to a unique value proposition: streamlined pathways combining national health insurance data, real-world validation environments, and expedited regulatory processes—all designed to reduce time-to-market before international expansion.
Wonju residents will experience medical AI integration across the care continuum, from hospital diagnostics to home monitoring, positioning them as both beneficiaries and contributors to the evidence base supporting these technologies.
Whether this ambitious "city-as-testbed" model can deliver on its promise to accelerate medical AI commercialization remains to be seen, but it represents a novel approach to one of health tech's persistent challenges: bridging the gap between laboratory development and real-world clinical deployment.
