Digital Therapy Breaks New Ground as First FDA-Authorized Treatment for Schizophrenia's Most Debilitating Symptoms
Click Therapeutics' smartphone-delivered intervention targets negative symptoms that have long eluded effective treatment, demonstrating 62% improvement over control in pivotal trial.

The FDA has cleared a prescription digital therapeutic that addresses one of psychiatry's most stubborn challenges: the negative symptoms of schizophrenia that leave patients withdrawn, unmotivated, and socially isolated even when hallucinations and delusions are controlled.
Motivista (CT-155), developed by Click Therapeutics, received 510(k) clearance as an adjunct to clinician-managed outpatient antipsychotic therapy for adults with schizophrenia whose positive symptoms are clinically stable. The authorization marks the first time the FDA has cleared any treatment—digital or otherwise—specifically targeting negative symptoms including avolition, anhedonia, and asociality, which affect approximately 60% of schizophrenia patients, according to the company.
Pivotal Trial Shows Significant Symptom Reduction
The clearance was supported by results from the phase III CONVOKE study, which enrolled 457 adults with a mean age of 45.8 years (60.6% men) experiencing negative symptoms of schizophrenia. Patients using Motivista showed a 6.8-point improvement on the CAINS-MAP scale at 16 weeks, compared to a 4.2-point improvement in the control group—representing a 62% relative improvement (P=.0003).
The smartphone-delivered intervention requires approximately one hour per week over 16 weeks and employs CBT-based techniques including cognitive restructuring, behavioral activation, social skills training, and positive affect training. Adherence rates reached 70.4% in the treatment group and 76.5% in the control group at week 15.
Safety data showed serious adverse events in 1.3% of the treatment group (3 patients) versus 2.2% of the control group (5 patients), with none considered treatment-related by investigators.
Addressing an Unmet Clinical Need
"Motivista addresses a critical unmet need by targeting negative symptoms, which are often the most debilitating aspects of schizophrenia and have historically been difficult to treat," said Tam Dang-Tan, PhD, Vice President of Medical Affairs at Click Therapeutics.
The digital therapeutic is not appropriate for all patients. Contraindications include serious suicide risk, acute symptoms, patients not on antipsychotics, severe cognitive impairment that would prevent app use, and those under 18 years of age.
Navigating the Reimbursement Landscape
Click Therapeutics joins a growing field of FDA-authorized digital therapeutics for mental health conditions including major depressive disorder, generalized anxiety disorder, insomnia, and substance use disorders. However, Motivista represents the first such clearance specifically for schizophrenia.
The company is pursuing state-level Medicaid coverage and Medicare Part D protected class status as part of its reimbursement strategy. This approach reflects the reality that Medicaid covers a significant share of adults with schizophrenia, making state-level payer decisions critical to commercial viability.
The smartphone-based delivery model may help overcome traditional barriers to accessing psychosocial treatments, though patients still require concurrent antipsychotic management and clinical oversight to ensure appropriate use and monitoring.
